**Group Discount: Three or more attendees who register under one transaction are eligible for a $60 savings per registrant. All registrants must complete a registration form. For online registrants, please indicate the other members in your group in the Comments box of the recipient information page.

How to register:

• Online with a credit card. Confirmations will be sent by e-mail.
• Fax the registration form to 303.422.8894 with payment.

Cancellations:
Notification of cancellation must be submitted in writing.

Cancellations received on or before 15 April 2013 will be subject to a $250 cancellation charge. No refunds will be given for cancellations received after 15 April 2013. Registrants who do not attend the Annual Meeting and do not notify CSE headquarters in writing by 15 April 2013 deadline forfeit the full registration fee. Substitutions are permitted until 22 April 2013. Substitution requests must be received in writing at CSE headquarters by 22 April 2013.
